Who Is AXA Health?

AXA Health has been providing healthcare services in the UK since 1940, originally founded as a collaboration between doctors and hospitals to give ordinary people faster access to specialist care. Today it is one of the UK’s largest private medical insurers, covering over 250,000 SME members and holding a 5-star Defaqto rating for its Business Health product.

Their business health insurance is fully modular — unlike Bupa’s pre-built tiers, you start with a core and bolt on only the cover your team needs.

The Modular System: Build Your Own Policy

Most health insurers offer pre-built tiers. AXA works differently: you start with core hospital treatment and add individual modules based on what your business actually needs. A startup with five healthy staff in their twenties doesn’t pay for the same cover as a professional services firm that wants mental health support and dental. Each module has its own cost, so you can see exactly where your money goes and adjust at renewal.

AXA’s Business Health product covers SMEs with 1 to 249 employees. For businesses with 250 or more staff, they offer AXA Advance with additional occupational health screening and corporate wellbeing tools.

Broker’s View — Why Modular Matters

In my experience, AXA’s modular approach tends to produce very competitive quotes for businesses that know what they want. If you’re clear on priorities — say, strong cancer cover and digital GP access, but you don’t need therapies or dental — AXA lets you strip out everything else. That’s where the savings come from. The trade-off is that you need to understand what each module does, which is exactly where a broker adds value.

What Every AXA Business Plan Includes

AXA’s core “Treatment Option” forms the foundation of every business health insurance policy. It provides:

In-Patient and Day-Patient Treatment

If an employee needs to stay in hospital overnight or attend as a day patient, AXA covers diagnostic tests, specialist fees, hospital accommodation and ambulance transport in full — provided treatment takes place at an AXA-approved facility.

Outpatient Surgery and Scans

The core plan covers outpatient surgery with no yearly limit, along with CT, MRI and PET scans referred by a specialist. These must be carried out at AXA-approved hospitals and scanning centres.

Cancer Cover

AXA offers two levels of cancer cover. The NHS Cancer Support option means private cancer treatment isn’t covered but AXA will fund licensed medications the NHS won’t pay for. The Comprehensive Cancer Cover option provides full private cancer treatment — consultations, diagnostic tests, surgery, chemotherapy and all licensed medication paid in full. If cancer cover is a priority, comprehensive is the only option worth considering.

NHS Cash Benefit

Employees receive a cash payment if they choose to have certain treatments through the NHS rather than privately. This is a standard feature on all AXA business plans.

Digital GP: Doctor@Hand

Every member gets unlimited access to AXA’s 24/7 virtual GP service. Employees can book video or phone consultations lasting up to 20 minutes. If the GP identifies something that needs further investigation, they can refer directly to a specialist — provided the policy includes sufficient outpatient cover.

Three Levels of Outpatient Cover

Outpatient cover is where your choice has the biggest impact on both access and cost. The three tiers affect specialist consultations, diagnostic tests and fees.

Standard

Up to 2 specialist consultations per year. Full cover for diagnostic tests. No cover for specialist fees beyond consultations. The most affordable option — suitable for teams that primarily want hospital cover with basic diagnostic access.

Enhanced

Up to 4 specialist consultations per year. Full cover for diagnostic tests. No yearly limit on fees. A good middle ground for businesses that want reasonable outpatient access without paying for unlimited cover.

Full

No yearly limits on specialist consultations, diagnostic tests or fees. The most comprehensive — and most expensive — option. If your team is likely to use outpatient services regularly, this removes any worry about hitting a cap mid-year.

Broker’s View — Outpatient Choice

The outpatient tier is the single biggest driver of premium cost after age. Most of my SME clients land on Enhanced — it provides enough consultations to be useful without the jump to Full pricing. If budget is tight, Standard with the Digital GP can work well for younger teams who are unlikely to need frequent specialist visits. See our guide on comprehensive vs basic cover for more on this trade-off.

Additional Modules You Can Add

Mental Health

Covers counselling, cognitive behavioural therapy and psychiatric treatment. This is an optional add-on, not included as standard — a key difference from Bupa, which includes mental health as standard.

Therapies

Access to physiotherapy, osteopathy, chiropractic, homeopathy and acupuncture as an outpatient. Up to 10 sessions per policy year with a GP referral required.

Dental & Optical

Cashback on dental treatment (80% of fees up to £400/year), prescribed glasses and contact lenses (80% up to £200/year) and up to £25 for an annual eye test.

Private GP

Face-to-face GP appointments with same-day, evening and weekend slots. Up to £500 per year for private consultations — useful for fast referrals when your team can’t wait for NHS GP availability.

Employee Assistance Programme

Confidential support from healthcare specialists covering mental health, life management advice and consumer rights help. Available alongside other health cover.

Travel Cover

European or worldwide emergency medical and travel insurance for employees travelling on business or leisure. Up to £10 million in emergency cover. Keeps all your team’s protection under one policy.

Extended Cover

AXA also offers an Extended Cover module that broadens the policy considerably. It covers treatment at hospitals outside AXA’s standard directory, pre-approved treatment outside the UK (up to the cost of equivalent UK treatment), access to a wider range of specialists, and — notably — cover for specific chronic conditions including asthma, diabetes, epilepsy, osteoarthritis, rheumatoid arthritis and heart valve problems. This is unusual in the market, as most private health insurers exclude chronic conditions entirely.

Broker’s View — Extended Cover

The chronic condition cover within Extended is worth a close look. If you have employees managing conditions like diabetes or arthritis, this module can provide support that most policies simply don’t offer. It adds to the premium, but for the right team it can shift the value equation.

No GP Referral Needed: AXA’s Direct Access Services

One of AXA’s standout features for employers is the suite of services that bypass the GP referral process entirely. In 2026, with NHS GP appointments often taking weeks to secure, this can make a significant difference to how quickly your employees get treated.

Working Body

Direct phone or video access to physiotherapists and osteopaths for musculoskeletal issues — neck, back, shoulder and joint problems. No GP referral needed. This is included for all members at no extra cost and is one of AXA’s most-used services.

Stronger Minds

Direct access to mental health support without needing a GP referral. Employees can speak to a professional about stress, anxiety, depression and other mental health concerns. Available when the mental health module is included.

Fast Track Appointments

For members who have a GP referral and are waiting for treatment, AXA’s Fast Track team matches them with an appropriate specialist from the network and arranges the appointment. Requires outpatient cover.

Specialist Help

A 24/7 helpline where employees can speak to pharmacists about medication, nurses about treatment options, counsellors for emotional support, and midwives for pregnancy-related queries.

Exclusions and Limitations

Like all private health insurance, AXA’s business plans have exclusions. Standard exclusions include routine pregnancy and childbirth, cosmetic surgery, and chronic conditions (unless you have Extended Cover). Outpatient medication and dressings are also excluded — employees would need to see their NHS GP for prescriptions.

Pre-existing conditions are handled through the underwriting method you choose. With moratorium underwriting on AXA Business Health, AXA looks back at the five years before the employee joined. A pre-existing condition may become eligible for cover once the employee has been a member for two consecutive years and has gone 12 consecutive months trouble-free from that condition. With full medical underwriting, each employee’s history is assessed individually and specific exclusions applied.

Key Advantage — Chronic Cover via Extended Module

Unusually in the UK market, AXA offers an Extended Cover module that can fund treatment for specific named chronic conditions, including asthma, diabetes, epilepsy, osteoarthritis, rheumatoid arthritis and heart valve problems. Most other major UK insurers exclude chronic conditions entirely, so this is one of the genuine differentiators of AXA Business Health if your team includes employees managing these conditions.

How Much Does AXA Business Health Insurance Cost?

Costs vary widely depending on what you include. Premiums are age-rated and influenced by location, team size, cover level and excess. For a broader look at health insurance costs, see our pricing guide.

Indicative Monthly Cost Per Employee (2026)

These are typical ranges for non-smoking employees outside Central London. Your actual quote will vary.

Core Only (Hospital + Scans)

£30 – £55 / month (age 30–40)
In-patient, day-patient, surgery and diagnostic scans. No outpatient consultations or therapies. The most affordable option.

Core + Enhanced Outpatient

£50 – £90 / month (age 30–40)
Adds 4 specialist consultations per year and full diagnostic tests. The most popular configuration for SMEs.

Core + Full Outpatient + Mental Health

£75 – £130 / month (age 30–40)
Unlimited outpatient access plus mental health module. Comparable to mid-tier Bupa or Vitality plans.

Comprehensive (All Modules)

£100 – £180+ / month (age 30–40)
Full outpatient, mental health, therapies, dental, optical and extended cover. Premium end, though in my experience it tends to come in below a fully-loaded Bupa Complete policy.

Ways to Reduce Your AXA Premium

Choose the Guided Option. Instead of picking any consultant, employees use AXA’s Fast Track service which recommends up to three specialists. This can reduce premiums by 20% or more.

Set a higher excess. AXA charges the excess once per employee per policy year — not per claim. A £250 or £500 excess can make a meaningful difference to monthly costs while remaining affordable for staff.

Start lean, add later. Begin with core cover and Enhanced outpatient. You can add mental health, therapies or dental at renewal if your team values them. This phased approach keeps year-one costs manageable.

Broker’s View — Tax Treatment

Employer-paid health insurance premiums are a deductible business expense, reducing your corporation tax bill. The benefit is taxable for employees via P11D, but even after tax, it’s typically much cheaper than buying an equivalent individual policy. For directors of owner-managed businesses, this is often a cost-effective route to private healthcare — though you should check with your accountant for your specific circumstances. If you also need life insurance or income protection, these can often be arranged alongside your health cover.

AXA vs Bupa: Bupa bundles more into its core — mental health, broader cancer cover and a larger hospital network at the top tier. AXA gives you more control over what you pay for. If your priority is comprehensive clinical cover and budget is secondary, Bupa may suit better. If you want to build a lean, targeted policy and keep costs down, AXA is typically the stronger option.

AXA vs Aviva: Aviva is often the most affordable option with straightforward tiered plans. AXA is more customisable and typically stronger on direct access services. Aviva’s simplicity suits businesses that want an easy setup; AXA suits those willing to spend time designing the right cover.

AXA vs Vitality: Vitality’s rewards programme (Apple Watch, gym discounts, activity tracking) is the most developed in the UK PMI market and appeals to health-conscious teams. AXA doesn’t offer lifestyle rewards but delivers more granular control over clinical cover. If your team values wellbeing incentives, Vitality wins. If they value practical healthcare access, AXA is typically stronger. For a fuller comparison across all providers, see our best business health insurance providers guide.

Who Is AXA Business Health Insurance Best For?

AXA Is a Strong Fit If…

You want maximum cost control. You only pay for the modules your team uses — few other major insurers offer this level of granularity.

Your team includes employees managing chronic conditions. AXA’s optional Extended Cover module can fund treatment for specific named chronic conditions (such as asthma, diabetes, epilepsy and osteoarthritis), which most other major UK insurers exclude entirely.

MSK issues are common in your workforce. Working Body gives direct access to physiotherapists without a GP referral — particularly useful for physical or desk-based teams dealing with back, neck and shoulder problems.

You’re a single-person company. AXA is one of the few insurers offering business health insurance from just 1 employee, making it accessible to sole directors.

Consider Alternatives If…

Mental health cover is a non-negotiable. Bupa includes mental health as standard with no chronic reclassification. AXA makes it an optional paid module, and the depth of cover isn’t as extensive.

You want everything included from day one. If you prefer a single package rather than assembling modules, Bupa’s tiered approach or Aviva’s simpler structure may save you time.

You want lifestyle rewards to engage your team. Vitality’s rewards programme is hard to beat on engagement. AXA offers gym discounts but nothing comparable to Vitality’s activity-linked incentives.

You have a history of cancer. AXA can apply blanket exclusions for past cancer diagnoses, even after years of remission. Some other UK insurers will consider cancer history case-by-case under full medical underwriting, so it is worth comparing offers if cancer history is a factor for your team. Common Questions About AXA Business Health Insurance

AXA’s Business Health plan covers SMEs with 1 to 249 employees — making it one of the few major insurers that accept single-employee businesses. For companies with 250 or more staff, AXA offers its Advance corporate product with additional occupational health and wellbeing tools.

All AXA Business Health plans include in-patient and day-patient treatment, full diagnostic scans referred by a specialist, 24/7 digital GP access via Doctor@Hand, the Working Body musculoskeletal service, and Specialist Help. Cancer cover level and outpatient access are chosen by the employer when building the policy.

Instead of choosing a pre-built tier, you start with core hospital treatment and add individual modules — outpatient cover, mental health, therapies, dental, optical, travel or extended cover. Each module has its own cost, so you only pay for what your team needs. This makes AXA one of the more customisable options in the market.

Mental health is an optional module, not included as standard. When added, it covers counselling, CBT and psychiatric treatment. AXA also offers the Stronger Minds direct access service for mental health concerns without needing a GP referral. If mental health is a top priority, compare with Bupa, which includes it as standard.

On AXA Business Health, AXA looks back at the five years before the employee joined — in line with Bupa, Aviva and Vitality. A pre-existing condition may then become eligible for cover once the employee has been a member for two consecutive years and has gone 12 consecutive months trouble-free from that condition. Full medical underwriting is available as an alternative, where each employee’s medical history is assessed individually before cover starts.

Costs depend on team size, average age, location, cover modules and excess level. AXA’s modular approach means you can start from around £30–55 per employee per month for core-only cover and scale up as needed. A broker can provide a tailored comparison quote at no cost to you.
