What is comprehensive health insurance?

Comprehensive health insurance describes a broader benefit package, not a universal standard. These labels are not standardised across insurers, so check the actual outpatient, cancer, mental-health, hospital and other benefits on the named product.

Comprehensive vs basic health insurance: what is the difference?

These labels are not standardised across insurers
AreaMore limited coverBroader cover
Hospital treatmentMay focus on eligible inpatient and day-patient treatment. Some modular products differ.May add outpatient diagnosis and other benefits.
Consultations and testsCan be excluded or subject to a monetary allowance. Can have a higher allowance or full eligible cover.
Cancer and mental healthCheck each separately; neither is universally included in every basic plan.Still check options, exclusions and treatment limits.
Medical historyUnderwriting and personal exclusions apply.More benefits do not automatically cover previous conditions.

Compare the actual product and schedule. For example, the current AXA Health Plan is modular, so assumptions about a universal “basic package” can mislead.

How to Compare Quotes

  1. Decide which treatment and diagnostic benefits matter to you.
  2. Request quotes using the same excess, hospital access and benefits.
  3. Check the proposed underwriting and exclusions.
  4. Confirm acceptance and the start date before cancelling any existing cover.

Paying a premium does not make every treatment immediately eligible. The agreed start date, medical-history terms and claim rules still apply.

How Cover Works in Practice

What happens from a symptom to treatment?

This is an illustration of the questions to ask at each stage, not a promise that any particular stage is paid for.

Questions to ask at each stage of care
StageQuestion to ask
First consultationIs a specialist consultation before diagnosis covered? Is a referral required?
Diagnostic testIs this test included, and does it use an outpatient allowance?
Hospital treatmentIs the procedure eligible, and are the hospital and consultant recognised?
Follow-upWhich consultations, rehabilitation or therapy are included afterwards?

Products divide these stages differently. The AXA Health Plan’s Inpatient and day patient care option does not include the outpatient consultations and diagnostic tests that come before admission; its Outpatient diagnosis and care option covers that part of the journey. Check how your quoted plan splits them (AXA inpatient option, outpatient option), and remember that benefit breadth is a separate question from underwriting: “comprehensive” does not mean existing conditions are covered.

The insurer pays eligible costs under the contract. You normally need to follow its referral and authorisation process before booking treatment. The policy may require a network specialist or a particular hospital list.

Check whether diagnosis is covered before hospital admission. Otherwise you may need to use the NHS or pay yourself to reach a diagnosis before an eligible operation can be authorised.

What Might a Basic Plan Cover?

A treatment-focused plan can cover eligible hospital admission and day-patient procedures while limiting outpatient diagnosis. Other products are modular, allowing you to choose different starting benefits. Read the schedule rather than relying on the word “basic”.

Major scans, cancer treatment, mental-health support and physiotherapy can sit in different parts of each insurer’s product. For example, Vitality includes limited Talking Therapy and Priority Physio support, but routine outpatient diagnosis is an option. Vitality’s core-cover summary.

Choosing cover

What does comprehensive health insurance cover?

A broader package may include outpatient consultations and tests, additional therapies or mental-health treatment. It can reduce reliance on NHS diagnosis, but does not guarantee immediate access or payment for every service.

Ask whether the outpatient allowance is shared by consultations, tests and therapies, whether major scans sit outside it and which referral rules apply. Dental, optical, screening and travel may still be separate even on a plan described as comprehensive.

Choosing cover

What Is Still Excluded?

Broader cover does not cancel medical-history exclusions. Routine maternity, cosmetic treatment and ongoing management of chronic illness are commonly restricted, subject to specific benefits and exceptions.

Cancer and mental-health wording can qualify a general chronic-condition exclusion. Check the exact product rather than assuming every long-term diagnosis is permanently outside cover. Bupa’s underwriting guidance.

Underwriting Is a Separate Choice

Basic does not mean moratorium, and comprehensive does not mean full medical underwriting. Benefit selection and medical-history assessment are separate decisions.

Under full medical underwriting, the insurer assesses disclosed history and sets written terms. A moratorium uses the policy’s look-back and qualifying rules; symptoms, medication, treatment and advice can all matter. The look-back is not universally five years: the current AXA Health Plan uses three years for its general rule, with separate specified-condition restrictions.

When switching, ask whether continued underwriting is available and whether new exclusions will apply. AXA’s current underwriting explanation.

Inpatient, Day-patient and Outpatient: The Difference

  • Inpatient: admitted to hospital with an overnight stay.
  • Day-patient: admitted for treatment or recovery without staying overnight.
  • Outpatient: seen without admission, for example for a consultation or diagnostic test.

Check the policy’s definitions and benefits for each setting. A treatment being medically necessary does not itself establish that the chosen cover pays for it.

How Much More Does Comprehensive Cover Cost?

Ask for two quotes from the same insurer with the same personal details, underwriting, excess and hospital access, changing only the benefits you want to compare.

As an attributed example only, Bupa publishes £44.90 a month for a UK non-smoker under 50 with Comprehensive cover, £500 outpatient allowance, £500 excess, Extended Choice and full cancer cover, for one person. These are May 2026 figures on a page updated 3 July 2026, checked 15 September 2026. They do not measure the price difference between basic and comprehensive plans. Bupa’s source and assumptions.

Basic and Comprehensive Cover FAQs

What is comprehensive health insurance?

It is a description of a broader benefit package, not a universal standard. Check the actual outpatient, cancer, mental-health, hospital and other benefits.

Does comprehensive cover include previous conditions?

Not automatically. Underwriting and personal exclusions are separate from the number of benefits chosen.

Is cancer always included in basic cover?

No. Some products include it while others make it an option or offer different levels. Check the named product.

Does a basic plan require moratorium underwriting?

No. Benefit level does not determine the underwriting method. Check the options the insurer offers.

How much more does comprehensive cover cost?

Request matching quotes changing only the benefits. The difference depends on the actual cover and personal details.

Can I upgrade after I develop symptoms?

Do not assume new benefits will cover symptoms that have already started. The insurer may apply underwriting or restrictions to an upgrade.

Why Use a Broker

Benefits of Working with a Health Insurance Broker

Choosing the right health insurance can be confusing, especially with so many options available. That's where a health insurance broker can help. A broker works with a number of insurers and can price the same cover across them, so you compare like with like.

Which Providers We Compare

When you work with a health insurance broker, you gain access to a wide range of insurance providers. The insurers we compare include Bupa, AXA, Aviva, Vitality, WPA, and The Exeter. Ask which insurers are included in any comparison you are shown.

Matching Cover to Your Needs

A broker's job is to match the cover to your needs and budget. Whether you need basic coverage or a comprehensive plan with extra benefits, a broker can guide you to the right choice. They can also help explain the differences between plans, so you understand what you're getting.

Saving Time and Effort

Instead of spending hours researching and comparing different health insurance plans on your own, a broker does the work for you. They have the knowledge and experience to quickly identify the options that fit your needs. That saves you time comparing quotes yourself.
