Private Health Insurance in Northern Ireland

Private health insurance is available in Northern Ireland and can sit alongside Health and Social Care Northern Ireland (HSC). The practical questions are which private hospitals and specialists your policy gives you access to, how your medical history is treated and whether the cover is worth the premium.

We compare suitable policies from leading UK health insurers available to us and explain the differences in plain English — including hospital access, outpatient cover, excess and underwriting.

Northern Ireland has a smaller private hospital market than many parts of Great Britain, so the cheapest quote is not necessarily the most useful. We would start with the hospitals and specialists you can realistically use, then compare benefits, underwriting and price.

Local Access

Check the exact private hospital list and recognised specialist network before comparing premiums.

Medical History

New policies usually apply underwriting to pre-existing conditions; switching may work differently.

Outpatient Cover

Consultations, scans and diagnostics can be full, capped or restricted depending on the plan.

Price

Age, postcode, smoking status, excess, benefits and insurer all influence the premium.

How does private health insurance work in Northern Ireland?

Private medical insurance (PMI) gives you an additional route to eligible private diagnosis and treatment. You keep full access to HSC and continue to use public services for emergencies and treatment that sits outside your private policy.

A policy is not a promise that every private hospital, consultant or treatment is covered. The insurer's hospital list, recognised specialist rules, outpatient benefits, excess, exclusions and underwriting all matter.

Bupa's Northern Ireland guidance describes private health insurance as cover that can be used alongside HSC, while AXA Health and Aviva also publish products or guidance showing health insurance availability in Northern Ireland. The exact products available through an intermediary can differ from direct-to-consumer products, so the comparison should be made on the policy you are actually being offered.

The useful starting question

Where would you actually want to be treated? In Northern Ireland, hospital access can have more practical impact than a small difference in monthly premium.

What do Northern Ireland's latest waiting-time figures show?

The Northern Ireland Department of Health published its latest quarterly waiting-time statistics on 3 September 2026, covering the position at 30 June 2026.

504,537
patients waiting for a first consultant-led outpatient appointment
59.1 weeks
median wait for a first consultant-led outpatient appointment
231,145
patients waiting for a diagnostic test

The same release says 53.7% of those waiting for a first outpatient appointment had waited more than 52 weeks, while 35.0% of those waiting for a diagnostic test had waited more than 26 weeks.

These figures help explain why some people consider private cover, but they do not mean buying insurance today will pay for a condition that is already symptomatic, under investigation or on an HSC waiting list.

Important data note

The Department states that figures sourced from the encompass electronic patient record are “official statistics in development” and are not directly comparable with legacy pre-encompass data.

How much does private health insurance cost in Northern Ireland?

There is no single market-wide Northern Ireland premium. Your price depends on age, postcode, smoking status, insurer, hospital access, outpatient benefits, excess and other options.

For a transparent benchmark, Bupa publishes the following May 2026 non-smoker examples for one person in selected Northern Ireland cities. The example configuration is Comprehensive cover with a £500 outpatient allowance, £500 excess, extended-choice hospital access and full cancer cover.

Age bandBupa example monthly premiumImportant context

Under 40£24.34Bupa example, non-smoker

Which private hospitals can you use in Northern Ireland?

It depends on your policy. The Private Healthcare Information Network (PHIN) reported 23,000 admissions across Northern Ireland's 10 private hospitals in 2025. The independent market is therefore meaningful, but smaller than the market in England.

PHIN profiles show consultants working at facilities including Kingsbridge Private Hospital Belfast, Ulster Independent Clinic, Kingsbridge Private Hospital Northwest in Ballykelly and other Northern Ireland clinics. That does not mean every insurer or every policy covers every facility.

Belfast

Kingsbridge Private Hospital

A major private facility on Lisburn Road. Check that the hospital, consultant and proposed treatment are recognised under your policy.

Belfast

Ulster Independent Clinic

A private hospital on Stranmillis Road. Insurer recognition and consultant fee rules still apply.

North West

Kingsbridge Northwest

A Ballykelly facility that can be relevant for people outside Belfast, subject to insurer and treatment recognition.

Why the hospital list matters

Bupa tells Northern Ireland customers to check they can access private facilities nearby, particularly in rural areas. Aviva also offers a reduced Signature hospital option specifically for Scotland and Northern Ireland. The structure of the hospital list can therefore materially affect both usefulness and price.

What should you compare before choosing NI health insurance?

01

Hospital access

Which Belfast and regional facilities are actually on the plan's recognised list?

02

Outpatient benefits

Are consultations, diagnostics and scans covered in full, capped or restricted?

03

Consultant route

Does the policy use guided care, open referral or broader recognised-consultant choice?

04

Excess

How much could you contribute to a claim and how often can the excess apply?

05

Cancer & mental health

Compare the actual wording, pathways and limits rather than relying on headline ticks.

06

Underwriting

Understand what is excluded before judging whether one policy is cheaper than another.

Which insurers offer health insurance in Northern Ireland?

There is no need to pick an insurer from a brand list before you have checked the policy. Bupa and AXA Health both publish Northern Ireland-specific consumer guidance, and Aviva's individual health insurance defines UK cover as including Northern Ireland. Other insurers may also have options, subject to product eligibility and distribution.

Our role is to compare the specific products available to us for your circumstances, rather than assume every insurer or product is suitable.

What happens to pre-existing conditions and ongoing treatment?

A new individual policy is generally designed for eligible new conditions arising after cover starts. If you already have symptoms, have received advice or treatment, are undergoing tests or have been referred, the insurer may treat that issue as pre-existing under its underwriting rules.

That is especially important if an HSC wait is the reason you are considering cover. Buying PMI after the referral has been made will not usually turn that existing condition into a new insured claim.

Moratorium vs full medical underwriting

Under full medical underwriting (FMU), medical information is considered at application and exclusions or terms are confirmed. Under a moratorium, the insurer applies its published look-back and symptom-free rules to prior conditions rather than fully underwriting each one at application. Exact rules vary by provider.

Already insured and thinking of switching?

Do not assume a new policy must start from scratch. Depending on eligibility and insurer acceptance, switch underwriting or continued moratorium terms may preserve some existing underwriting treatment. Do not cancel current cover until the new insurer's written underwriting terms are confirmed and acceptable.

See our guides to pre-existing conditions, moratorium vs FMU and switching health insurance.

Can your policy cover treatment in Great Britain?

Potentially, yes — but the exact hospital and treatment must be eligible under your policy. Some people in Northern Ireland may consider travelling to Great Britain for a particular specialist or treatment where their policy allows it.

Do not assume travel and accommodation are part of the insured benefit. Bupa's Northern Ireland guidance says its health insurance is unlikely to cover travel costs and accommodation for receiving treatment, apart from recognised facility charges connected with eligible inpatient treatment.

Before you travel

Ask the insurer to confirm the hospital, consultant, treatment and authorisation in writing, and separately confirm whether any travel or accommodation costs are payable.

The six questions we would answer before you buy

Which private hospitals can you realistically reach from home?
Do you want full outpatient diagnosis or would a cap be enough?
What underwriting will apply to each person being covered?
Could treatment in Great Britain be useful for specialist access?
What excess are you comfortable paying if you claim?
If switching, what medical-history terms must be preserved?

Northern Ireland private health insurance FAQs

Yes. Private medical insurance is available in Northern Ireland and can be used alongside HSC. The hospitals, specialists and benefits available depend on the insurer and policy you choose.

There is no single market-wide price. Premiums vary with age, postcode, smoking status, insurer, hospital access, outpatient benefits, excess and other choices. The Bupa figures shown above are one insurer's examples, not market averages.

It depends on your insurer's hospital list and whether the hospital and consultant are recognised for the treatment. PHIN profiles show facilities including Kingsbridge Private Hospital Belfast and Ulster Independent Clinic. Always check the policy before booking.

Usually not on a new individual policy if the condition was already symptomatic, under investigation, treated or referred before cover started. The exact outcome depends on underwriting and policy terms.

Potentially. Switching terms, continued moratorium or other continuity arrangements may be available subject to eligibility and acceptance. Confirm the new insurer's written underwriting outcome before cancelling existing cover.

Some policies can cover eligible treatment at recognised facilities elsewhere in the UK. Hospital lists and authorisation rules apply, and travel or accommodation costs should be checked separately.
