The Short Answer

Which Private Health Insurance Is Best in the UK?

There is no defensible one-to-six league table that works for every person. A policy can be excellent for mental health but wrong for your preferred hospital; cheap at outset but less attractive after a claim; or generous on one benefit while restricting another.

For individuals and families, the six insurers we most often compare are Bupa, WPA, The Exeter, Vitality, Aviva and AXA Health. We judge them on the actual terms that affect a customer: hospital and referral access, outpatient cover, cancer, mental health, underwriting, excess or cost sharing, renewal mechanics and family/age criteria.

Current NHS context

NHS England figures published on 10 September 2026 show the elective waiting list rose to 7.3 million referral-to-treatment pathways in July 2026, with 65.4% waiting up to 18 weeks. These are pathways, not unique patients. Private medical insurance is an additional route to eligible private care; it does not replace the NHS.

This is a decision guide, not a scorecard. “Worth comparing when” describes where we think a provider deserves a closer look; it is not a claim that the insurer is universally best.

BupaDirect Access for eligible cancer, mental health and MSK concerns; Bupa makes a strong mental-health proposition.Mental health and streamlined access are high priorities. The exact outpatient, hospital/referral and benefit limits on the plan quoted.

Independent Evidence

What Do Recent Claimants Say?

Independent customer evidence is useful, but it should not be confused with a comparison of policy wording. In April 2026, Which? surveyed 931 private medical insurance customers who had made a claim in the previous year.

Which? customer scores, 2026

WPA 86% · Westfield Health 80% · Bupa 77% · AXA Health 76% · Aviva 74% · Saga 72% · Vitality 72%.

WPA was the only provider in the survey named a Which? Recommended Provider. Which? explicitly says its analysis is not a comparison of the cover levels in insurers' policies. We therefore use this as evidence about recent customer/claims experience, not as proof that WPA has the best cover or price for everyone.

Our Method

How We Compare Private Health Insurance

My Health Protected does not award an arbitrary overall score. We compare the material terms on the actual options available for the client, then explain the trade-offs.

Hospital and referral access: can you use the hospitals and clinicians that matter to you, and is the route guided or more open?

Outpatient cover: are consultations, diagnostics and scans fully covered, capped, shared with other benefits or excluded?

Cancer and mental health: what is included, what requires an option, and what pathways or limits apply?

Underwriting: how will medical history be treated under FMU, moratorium or continued underwriting?

Claims and renewal mechanics: excess, cost sharing, NCD structure and other factors that can affect future premiums.

Family and eligibility: age rules, family structures and benefit arrangements can change which insurer is suitable.

Price comes last in the comparison, once the benefits and underwriting are close enough to be meaningfully compared.

Provider Review

Bupa Health Insurance

Bupa is worth comparing when mental health and access pathways are important. Bupa states that it covers more mental health conditions than any other leading UK insurer, and most of its health insurance options include Direct Access for eligible cancer, mental health and muscle, bone and joint concerns.

What stands out

Direct Access can allow eligible members to contact Bupa about specified symptoms before seeing a GP. Bupa checks the issue is covered and can guide the member to next steps or, where appropriate, a specialist. The precise pathway still depends on the member's cover, symptoms and age criteria.

The useful comparison is not “Bupa versus everyone” in the abstract. Check the exact outpatient allowance, referral route, hospital access, excess and underwriting outcome on the version of cover being quoted.

Worth comparing when: mental health and direct-access pathways matter
Provider Review

AXA Health Insurance

For broker-arranged individual cover, the AXA products to compare are Personal Health and Health For You. AXA's separate AXA Health Plan is sold direct and has different benefits and underwriting wording, so its three-year moratorium should not be used to describe an AXA quote arranged through My Health Protected.

What stands out

Both broker plans use a five-year moratorium look-back and current intermediary material describes two continuous trouble-free years after joining before an eligible condition may regain cover, subject to the policy rules. Personal Health can use AXA's Guided Option and includes eligible outpatient CT, MRI and PET scans in core cover. Health For You is not Guided, uses its own hospital/specialist list, and has different therapies and mental-health limits.

Both Personal Health and Health For You can use Comprehensive Cancer Cover or NHS Cancer Support. If AXA is on your shortlist, compare the actual product name, outpatient level, hospital route, cancer option, therapies, mental-health cover, excess and underwriting terms rather than relying on general AXA marketing.

Worth comparing when: the Personal Health or Health For You benefits fit your priorities
Provider Review

Aviva Health Insurance

Aviva's Healthier Solutions is worth comparing when you want several ways to adjust the premium without starting from a completely different product. Its current core includes specialist consultations/treatment, diagnostics, hospital charges, cancer cover and outpatient mental health treatment.

What stands out

Aviva lets customers change cover using options including reduced outpatient cover, a six-week NHS option, member excesses and different hospital arrangements. Current excess choices run from £100 to £5,000. MyHealthCounts can provide a discount of up to 15% on a participating member's proportion of the renewal premium, subject to its terms and availability.

Premium-saving options are only good value if you understand what you are giving up. Compare the final hospital, outpatient and treatment structure rather than assuming the cheapest configuration is the best one.

Worth comparing when: you want several levers to balance cover and cost
Provider Review

Vitality Health Insurance

Vitality is worth comparing if you actively value wellbeing rewards alongside the insurance. Its current Core Cover includes eligible hospital treatment, cancer care, up to eight Talking Therapy sessions and up to six Priority Physio sessions each plan year.

What stands out

The Vitality proposition combines insured medical benefits with a wider rewards programme. For some customers that is a genuine advantage; for others it is secondary. We compare the insured benefits first, then treat rewards as an additional feature rather than letting them drive the recommendation.

Vitality states that eligible cancer treatment costs are covered in full at hospitals included in the plan. As with any insurer, the hospital option, outpatient structure and underwriting still need to fit the individual.

Worth comparing when: wellbeing rewards are genuinely valuable to you
Provider Review

WPA Health Insurance

WPA is worth comparing for customers who care about service experience and flexible cover. Its Complete Health product uses core benefits plus configurable options, and WPA says members have access to more than 1,000 hospitals, clinics and scanning centres, with premium hospitals available as an additional option.

What stands out

WPA was the only Which? Recommended Provider in Which?'s 2026 private medical insurance review and achieved an 86% customer score among recent claimants in that survey. WPA also allows recognised specialist choice within its rules and uses Fee Agreed information to help members understand potential fee shortfalls.

For new Complete Health members aged 66 and over, WPA's current brochure states that a minimum £500 excess applies and the policy must use Full Medical Underwriting. That is exactly the kind of eligibility detail that can make one provider suitable for one client and less suitable for another.

Worth comparing when: service evidence and flexible benefit choices matter
Provider Review

The Exeter Health Insurance

The Exeter's Health+ is worth comparing if you want a transparent No Claims Discount structure and the option to choose between traditional and lower-cost guided cover.

What stands out

Each member has their own NCD. The Exeter's current policy document says total claims paid up to £300 in the review period leave that member at the same NCD level; £300.01–£1,000 moves down one level, £1,000.01–£2,000 two levels, and more than £2,000 three levels. A member never drops more than three levels in one policy year.

Traditional Health+ lets the member choose from hospital lists and named specialists within the policy rules; guided cover uses an open referral and a shortlist of suitable specialists. That choice affects both flexibility and price.

Worth comparing when: NCD transparency and guided-vs-traditional choice matter
Decision Guide

Which Health Insurer Is Worth Comparing First?

Mental health

Bupa

Strong first comparison when broad mental-health support and Direct Access matter.

Claims experience

WPA

Independent Which? claimant evidence is particularly strong in 2026.

Cost controls

Aviva

Useful range of excess, outpatient, NHS-wait and hospital choices.

Wellbeing

Vitality

Worth comparing if you will actually use the rewards and activity proposition.

Modular cover

AXA Health

Current public Health Plan is built around configurable cover options.

NCD clarity

The Exeter

Graduated per-member claims impact can appeal if renewal mechanics matter.

These are starting points, not automatic recommendations. Hospital access and underwriting can overturn the apparent “best” choice very quickly.

Price

Which Private Health Insurance Is Best Value?

There is no insurer that is reliably cheapest for every person. Age, postcode, hospital access, outpatient cover, excess, family structure, underwriting and insurer pricing all matter.

That is why we have removed generic age-based prices from this comparison page. A quoted number without a postcode, excess, hospital option and benefit level can be more misleading than useful.

For detailed pricing evidence, see our average cost of private health insurance guide. When comparing insurers, line up the benefits first and then compare the premium.

MHP rule of thumb

If one quote is dramatically cheaper, find out what changed. Often the difference is hospital access, outpatient cover, referral route, underwriting or excess rather than the insurer simply being “better value”.

Switching

Can You Switch Insurer Without Losing Cover?

Sometimes — but never assume it. A new insurer may offer continued underwriting or switch terms, subject to its eligibility criteria and acceptance. The result can differ by insurer, medical history and existing policy.

Before cancelling the existing policy, confirm the new insurer's start date, underwriting basis, exclusions and any special terms in writing. A cheaper premium is not a saving if the switch worsens cover for a condition that matters to you.

Independent Advice

How My Health Protected Helps You Compare

My Health Protected is an independent, FCA-authorised health insurance broker. We compare suitable providers available to us and explain the material differences in cover and underwriting before making a recommendation.

We do not charge a separate broker fee for arranging private medical insurance. If you arrange a policy through us, we are paid commission by the insurer. Direct-only products or promotions can sometimes differ, so we do not claim every insurer route or price is identical.

Once cover is active, you deal directly with your insurer for claims. If things are not going smoothly, we can step in and help. We can also review your cover at renewal.

Common Questions

Best Private Health Insurance FAQs

There is no single best insurer for everyone. Bupa, WPA, The Exeter, Vitality, Aviva and AXA Health each have different strengths. The right choice depends on hospital access, outpatient cover, mental health, cancer, underwriting, excess or cost-sharing structure, renewal pricing and budget.

In Which?'s April 2026 survey of 931 private medical insurance customers who had made a claim in the previous year, WPA received the highest customer score at 86% and was the only Which? Recommended Provider. Which? states that this survey measures customer and claims experience rather than comparing policy cover levels or price.

Bupa is worth comparing when mental health is a priority because Bupa says it covers more mental health conditions than any other leading UK insurer and most of its health insurance options include Mental Health Direct Access. Vitality also includes up to eight Talking Therapy sessions within its current Core Cover. Always compare the exact policy terms for your circumstances.

There is no single reliable price that applies to everyone. Premiums vary with age, postcode, cover level, hospital access, outpatient benefits, excess, underwriting and insurer. Compare current quotes on the same benefit basis rather than relying on a generic internet average.

Yes. Hospital access can materially change both the usefulness and price of a policy. Before choosing an insurer, check that the hospitals or facilities you are likely to use are included on the exact hospital or guided-care option quoted.

Sometimes. Continued underwriting or switch terms may be available, subject to the new insurer's criteria and acceptance. Do not cancel an existing policy until the new insurer has confirmed the start date, underwriting basis and any exclusions or special terms.

Private medical insurance is primarily designed for new medical conditions. How prior conditions are treated depends on the underwriting basis and insurer. Full medical underwriting, moratorium underwriting and continued underwriting can produce different outcomes, so check the insurer's decision before buying or switching.

It depends on the insurer, benefit and condition. For example, most Bupa health insurance options include Direct Access for eligible cancer, mental health and muscle, bone and joint concerns, while other types of treatment may still require a referral. Check the claims pathway on the exact policy.
